LECOM’s Student Volunteer Survey has moved to:  portal.lecom.edu   (no “www”)

To report your volunteer activities, log onto the portal using your e-mail user name (format for current OMS2, OMS3, OMS4 and Pharm2, Pharm3 is either Jstudent or Joe.Student) and your e-mail password. OMS1 and Pharm1 students have been issued usernames and passwords for the portal.

1. Open http://portal.lecom.edu in your browser
2. Log onto the portal using your user name and password.
3. Go to the Surveys tab
4. Click the blue “Volunteer Survey” link on the Complete a Survey portlet.
5. Select your answers and click Next Page in the lower right of each screen.
6. The last page is Review and Submit.  If you did not complete any required fields, the form will provide links back to those questions.

The “Survey Reports” portlet will let you print a summary report of all your activity entered on this portal and will include all activity for 2008 reported on the old web site.  

PLEASE NOTE:  Data entered on this portal will NOT be instantly available.  We will process surveys overnight Monday thru Friday to make them available for the summary report.   The data you enter will be available after 9am the following day or on Monday if it was submitted on a weekend.
