The dates and times you request may not be available. This form is a REQUEST ONLY and DOES NOT imply availability. No parties can be booked without confirmation from our party specialists AND payment. Parties are NEVER confirmed until payment is processed so DO NOT send out invitations until we confirm your payment is processed. Invitations sent prior to confirmation and payment WILL NOT be honored.

Food Add-Ons
Food and beverage items may be added to your Fun Center, Bounce Zone or Golf & Games party. Please enter, in number format, how many items you want for each option. Enter 0 if you wish not to add-on. Birthday cakes, pizza or soda is not provided with each party. Pizza and soda may be added-on to each party. Each parent/guardian is responsible for providing the cake. ALL FINAL FOOD ORDERS MUST BE CONFIRMED BY INDIVIDUAL BOOKING PARTY BY 12 NOON ON THE WEDNESDAY PRIOR TO THE PARTY DATE BY CALLING WENDY OLSON AT (814) 838-3547.

Birthday Party Terms
1. Parties must be paid in full to book and are non-refundable. Parties may be re-scheduled only.
2. Food orders must be ordered a minimum of 48 hours in advance. Payment will be made on day of party.
3. All final food orders must be confirmed by individual booking party by 12 noon on the Wednesday prior to the party date by calling Wendy Olson at (814) 838-3547.
4. If your party is booked within 2 weeks of the party date, payment is accepted in cash or credit card only.
5. Due to the high volume of Birthday Party Requests, it will take 24 to 48 hours to respond to your request.
6. Forms submitted on Friday afternoon or over the weekend will be responded to before 5pm on the following Tuesday. Thank you for your patience.
