James Y. Lin, D.O., MS (Med Ed), Director
Dr. Lin is a Geriatric Medicine Specialist with MAE and Clinical Professor of geriatric Medicine at the Lake Erie College of Osteopathic Medicine. He received his medical degree from LECOM and completed his residency in Internal Medicine at Millcreek Community Hospital. In addition, he received his fellowship training at the University of Medicine and Dentistry of New Jersey. Dr. Lin is also board certified in Internal Medicine and is capable of providing comprehensive care for seniors in both the outpatient and inpatient setting.

Danielle M. Hansen, D.O., MS (Med Ed)
Dr. Hansen is an Internal Medicine Specialist with MAE and Clinical Assistant Professor of Internal Medicine at the Lake Erie College of Osteopathic Medicine. She received her medical degree from LECOM and completed her internship and residency at Millcreek Community Hospital. She is the 2008 recipient of the American College of Osteopathic Internists Humanism and Excellence in Teaching Award, and she is past recipient of the Alice J. Coppola Internal Medicine Award.
The LECOM Institute for Successful Aging offers primary care and consultative clinical services in geriatric medicine across the entire continuum of care. Our specialists work with primary care physicians, patients and families to encourage independence and shared decision-making in outlining the appropriate plan of care. Some of the programs we offer, include:
Memory Assessment Program (MAP)
There are many reasons for memory loss. Alzheimer’s disease is only one of them. Early evaluation is important. We offer assessment of memory loss through a multi-disciplinary team. The team meets with the patient and family to discuss the results of the evaluation and help plan for the future.
Comprehensive Geriatric Assessment Program
Primary care and consultative services are available for individuals who are enjoying good health as well as for those with health programs and chronic conditions. Since older individuals are the largest users of prescription drugs and often take multiple medications, appropriate medication management is important in avoiding adverse drug reactions that can compromise health and function. Our physicians are able to provide ongoing medical services and medication management, or they can provide consultation services and send the findings and recommendations back to the patient’s primary care physician. Our goal is to help individuals age successfully and maintain independence through utilizing their abilities in the best possible way.
Balance Disorders and Falls Prevention Program
Falling, or unsteady, slow or poorly balanced walking are common problems in elderly individuals. For older persons who have experience gait and balance disturbances or who have fallen or are fearful of falling, the Institute provides a comprehensive falls assessment program.
Expanded Mental Health Services
Depression and other mental health programs that result from stress, illness or medications are often troubling to elderly individuals. We offer geriatric psychiatric evaluation and consultation services in the office and at Millcreek Community Hospital’s Geriatric Rehabilitation Unit, select nursing homes and assisted living settings. In those instances where memory problems are also noted, neuropsychological testing is offered.
Osteoporosis Clinic and Geriatric Fracture Prevention Program
This clinic is the first of its kind in the region that is dedicated exclusively to the prevention of fractures in elderly patients by providing clinical evaluation and treatment of persons with osteoporosis and related bone disorders.
Nursing Homes and Assisted Living Facilities
Our physicians are on staff at several rehabilitation and long term care facilities. In addition, the Director of the Institute, James Y. Lin, D.O. is the Medical Director of MCH’s transitional care unit and the Millcreek Geriatric Education Center (Millcreek Manor).
The LECOM Institute for Successful Aging is located on the third floor of the LECOM John M. & Silvia Ferretti Medical Fitness & Wellness Center which is home of several specialty practices of Medical Associates of Erie. This unique location provides opportunity for an interdisciplinary approach with the resources available to provide a full spectrum of services including radiology and physical therapy.
